Housing Stabilization Services is a Medical Assistance benefit to help people with disabilities and seniors find and keep housing. Housing Stabilization Services are a state plan (not waiver). Home and Community-Based Services (HCBS). Three services are provided within Housing Stabilization Services:
• Housing Consultation - service to develop a person-centered
plan for people without MA case management services.
• Housing Transition - service that supports a person to find
housing.
• Housing Sustaining - service that supports a person to maintain
housing.
Relocation service coordination targeted case management (RSC-TCM):
A form of TCM that provides coordination of activities to help a person who resides in an eligible institution gain access to medical, social, educational, financial, housing and other services and supports that are necessary to move to the community.
Institution: For the purposes of this page, "institution" is defined as any of the following:
• Hospital
• Nursing facility (NF), including certified boarding care facility
• Intermediate care facility for persons with developmental
disabilities (ICF/DD)
• Regional treatment center (RTC) that provides inpatient services
to people who currently receive Medical Assistance (MA).
Bundled service that include six categories. (ICLS) services offer assistance and support for older adults who need reminders, cues, intermediate/moderate supervision or physical assistance to remain in their homes. (ICLS) covers assistance and support for people over 65 and older enrolled in Elderly Waiver (EW) or the Alternative Care (AC) program. It includes the following service categories
• Active Cognitive Support
• Adaptive Support Service
• Activities of Daily Living (ADL's)
• Health, Safety, and Wellness
• Community Engagement
• Household Management
Services providing support and training in community living service categories for people 18 and older who live in their own homes. Individualized home support can be provided in the person's own home or in a public community settings and either in person or remotely.
Instructional Services through which a person receives from a staff member on community living skills identified in a person's assessment (e.g., Mn CHOICES, long-term care consultation [LTCC]. Training includes skill building to acquire, retain, and improve the person's experience living in the community. Individual Home Supports are available for people in their own homes and on one of the following waivers:
• Brain Injury (BI)
• Community Alternative Care (CAC)
• Community Access for Disability Inclusion (CADI)
Promotes a healthy lifestyle. We are here to help solve problems/concerns a person may have related to daily living. We provide assurance, observe and reflect to address cognitive, and behavioral concerns. Furthermore, we are creative and will do everything possible for our clients. They will have access to every resource available to them or that they are eligible to receive. We will leave no rock unturned. Some services may require some patience as they are being processed.
