Community Outreach Services LLC promotes optimal health, safety, community living, and independence designed to address individual needs with support and assistive needs.

We collaborate with a person to arrange health care (e.g., physical, mental, chemical) meaningful activities, social services, meetings and appointments. Support for the person in designing and meeting individualized strategies to reach health, safety, and wellness goals.
